(A)  Failure to Furnish Sentencing Court with Accurate Information.  A sentence must be vacated where the prosecution fails to furnish accurate information to the court at the time of sentence.[378]  The prosecution must also correct errors and ensure “that the factual basis for the sentence will be both adequate and accurate, and that the record of the sentencing proceedings will accurately reflect relevant circumstances of the offense and characteristics of the defendant which were not disclosed during the guilt phase of the case . . . .”[379]
